Department of Communities, Disability Services - Local Coordination Program
As the Local Coordinator for Disability Services, I support people with a disability and their families/carers who live in the Shire of Waroona. I can provide information and advocacy and assist you to plan, organise and access supports and services.
I work from the Waroona Community Resource Centre (10 Henning St, Waroona) on Thursday mornings, but can make appointments on other days if more convenient for you.
From 1st October 2018, the National Disability Insurance Scheme (NDIS) will start to contact people who are registered with Local Coordination. The NDIS is the Australia-wide scheme that will be responsible for planning and funding disability services in Western Australia and gradually everyone in the Shire of Waroona will be transitioned to the NDIS. Until you have an NDIS plan in place, you will continue to receive the usual services and supports through your individual plan.
If you have a disability and are not registered with Local Coordination, you can now contact the NDIS directly to test your eligilibity for that scheme.
